VOLUNTEER APPLICATION

To apply for a volunteer position at OMSI, please download the appropriate application. Application forms are provided in either Adobe Acrobat or Microsoft Word formats. If you wish to fill out the application on your computer and email it to us, the Word version may be more convenient. To send the application via email you will need to first save it as a file on your computer and then attach it to an email.

Note: In response to requirements of our insurance company and concern for the safety for our visitors, OMSI has implemented a policy of fingerprinting all staff and volunteers who spend 15 percent or more of their time working with visitors. Our goal is to ensure that we safeguard children and other visitors to OMSI. Fingerprints will be run through state and federal databases, aiming to identify individuals who have been convicted of crimes that could pose a potential risk to OMSI's visitors. Fingerprinting will be conducted on site, prior to the start of employment or volunteering. OMSI appreciates your understanding in this matter.
